Sponsor Forms

The following form is to be used when obtaining donations for the 2008 PUT ride. It is very important that all information on the form is completely filled out before sending it in. Also, if you leave the form with the donor to send in, make sure all of the info is filled in so the treasurer does not have to track the supported member down after receiving it.
